Smart as you are, you have given us zero clue as to where you are. Go to the nearest FRO and grovel, apologize, and shed some tears. Pay fines, and ask them for a solution.

She should also contact her Embassy in New Delhi.

No choice. Nothing else to do.

You have to grovel a bit, sir this and sir that and please madam...

Knowing that you have done wrong and showing that you are genuinely afraid of the consequences and, please, can they possibly help you both is a good start.

Every day you avoid and are in a denial will not make the problem go away..will it ? Will all the intelligence higgs-boson as granted you, you fail to mention where you reside, and what is the nearest FRO/Sup. office ? Did your wife contact her Embassy ever ?

How long ago did your wife's visa expire? There is a big difference between weeks and months, and that is one reason why you should get on with this.

She contacts her Embassy for just in case she might need it and to see if she can get a new passport.
You and her meet the FRO (District SP of Police Office)/ FRRO in case of large Metros and do as advised above,

You still did not tell us which city/state/district you are in.

Is there something you are hiding ? Or are you a troll ? Politely twice if not more times you were asked which city/state, and you talk about medical visa !!! Pregnancy is not eligible for medical visa. It is not an illness or a disease ! The SP is correct, T visa cannot be extended. Delhi FRRO does not have jurisdiction outside of its defined area - MHA does.

Good luck and keep us updated !!!
