Dependent Visa query

I am getting married in FEB2013 and will be traveling to US for work in APRIL 2013. My wife doesn't have a passport yet.

My main concern is with my wife dependents visa:
1. For her dependent visa will she require my name in her passport and her name in mine?
2. Is it ok if her passport contains her maiden name in her passport or does she need to have updated name as per our marriage certificate.

Not required for H4 or L2.

Is she changed her name upon marriage - ie does the marriage certificate have her married name? In that case the dependant visa(which is dependant on her relationship to you - based on the marriage certificate) will also be issued in her married name. So the passport also needs to be in her married name.

As the marriage is in future - the best option would be to not change her name on marriage. Let her continue with her maiden name. Apply for the marriage certificate in her maiden name. In that case the name change is not required anywhere.

I would recommend not changing the name on marriage to any woman who has career aspirations. It causes a lot of needless hassles and doesnot have any practical benefits.

She wont be changing her first name. But last name will changed to mine which is "Pai", so does the marriage certificate will reflect "firstname Pai". So in this case her passport also needs to have "firstname Pai" or her previous last name will do?

Why does she want to change her last name? It is not an essential condition for being married

Passport always needs to be in her actual name. If she changes her name - ie changes her last name - then the passport also needs to be in the new name.

I know. But parents don't agree to that

So I guess this will be a reissue . In this case what address proof will be accepted since she will be staying at my place.

If the name change is mandatory (which repeating is not mandatory, and atleast now right away), then the time available might not be enough for the get-marriage certificate, passport reissue, get visa etc in the time you have.
