Renewal of passport under tatkal scheme - mulitple addresses in last one year

I need to renew my passport urgently. Its due to expire in June 2011. The only problem is that in the last one year my address has changed once. I do have address proof for both the addresses (bank statements). However, I am planning to get a verification certificate to support my tatkal application. Now, the Annexure-F has a clause - "Applicant has been staying at his/her address continuously for the last one year." Can this be edited to say that I have resided at two given addresses in the last one year? Will the passport office accept this?
If not, any other option to get a renewed passport urgently?

You must not hide/conceal any information to the passport office. Infact you have to fillup all addresses you have stayed in the past years. Verification will be done all the addresses (i think) and once over, you will get the passport.

You have to provide address proofs for addresses, proving that you have stayed there during so and so time.
