name is wrong in passport-please help

hi all,

please help me with this. i am planning to go on a vacation in jan 2013.
1. my name has v.v.v. initials. i was told to expand the name. i expanded the name but it was inputted wrongly in the passport. now i need to correct that. please advise me the right way to do it. passport was issued in 2007.
2. also i have entered my [COLOR="rgb(244, 164, 96)"]dad name as per my 10 class certificate[/COLOR]s (which is actually wrong in the certificates, but as it was in the certificate i had to use it). please suggest would it be a problem.

The Name in the passport is always the FULL EXPANDED form. No initials.

This is an OLD passport issued in 2007. So why the delay in getting it corrected? Irrespective of whose mistake it was (ie, yours while filling the form or theirs while printing), a new passport booklet may need to be issued. And what is extent of error? Is it just a spelling error or something more?
