Birth Certificate Issue for New Passport

Hello,

My son was born in 1993 and when we applied for the passport at the RPO, Chennai they mentioned that the name of the parents mentioned in the birth certificate doesnt tally with the real names.

During his birth his grand parents who are Illiterates just gave the last name of myself and my wife which they used to call at home and with that we got the birth certificate but in our passports we are having the full names with the surnames.

Now the Cantonment Board of St Thomas Mount,Chennai is not ready to change the names of the parents to their full names we are struggling for the past 2 years and the passport office is not issuing him the passport as the birth certificate doesnt have the full name of the parents.

Can any one suggest what we should do in this regard ? I am working in middle east and I am not able to bring my son with me for the past 2 years due to it.

for name change / correction / addition... whatever the case may be -
an affidavit is required... for this you will need a lawyer.
he will make the affidavit on the relevant stamp paper.

here are the steps:
1. Create an Affidivate stating that the name needs to be changed from to . [the lawyer will have the format]
2. Publish the name change in a regional newspaper - [in this case, a tamil paper]
3. Publish the name change in a local english newspaper.
5. Buy 3-4 copies of each newspaper that the name-change has been advertised. [having extra copies will never hurt, just in case you need it somewhere else]
6. Photocopy 3-4 copies of the affidavit as well - and have them notarised. [having extra copies will never hurt, just in case you need it somewhere else]

together with the application - you have to submit the original affidivate, both the paper clippings of the tamil newspaper and the english newspaper.

In my birth certificate my name is ABC DEF.
But I use the name ABCDEF n I applied for passport as the same name n I have same name in all the documents.
Will space between my name in birth certificate create any issue?

There should not be any problem. You may give it a try.

If there is any issue, please ask the remedy from the officials.

Problem in getting passport due to birth certificate

Hello

I was born on 29.10.1989 but my birth was not registered by my parents in the municipal committee.

Today when I tried to fetch the details from the municipal committee then they inform me that the birth is not registered in the records.

And I also tried to get the details from the hospital but the records of the hospital are also not available.

So kindly suggest me how Can I get the birth certificate for passport or any other document will fulfills the requirement of the passport.

You should visit the RPO and discuss the matter with the APO there. They may suggest you a way out.

It is very lengthy & hectic process to get birth certificate from MCG.I have gone through for my two sisters (1976 & 1983 Born) but finally get it. Where are you born i.e. at Hospital or at Home? If in hospital then try to get birth details from them.And if you born at home then you have to submit following documents:
1-Affidavit on RS-10/- Stamp Paper of your Father
2-Affidavit on Rs-10/- stamp papers from "Dai","Midwife" who has performed delivery process!!!
3-Affidavit on Rs-10/- stamp papers from two neighbors as witness who confirms about your birth!!!
4-Affidavit on Rs-10/-stamp paper from your Landlord if your family was leaving on rent during your birth!!!
5-True copies of Voter list which confirms about your family whereabouts during your birth!!!
6-If your father was in Government Job & posted on different location i.e. from your birth place- then "NOC" from Municipal Committee about that they do not have your name registered at their office!!!
7-If your mother was on job during your birth then proof of maternity leave taken from Organisation!!!
6-You have to submit birth details of your brothers & sisters too i.e.10th certificate!!!
All above documents you have to submit to MCG,Gurgaon!!!
After if you required Birth Certificate urgently then follow given:
Take documents from MCG personally &>submit to Government Hospital Gurgaon>Patwari(your area concerns)>Tehsildar>SDM>Government Hospital again>take application personally To Birth & Death registration Department,Chandigarh>Again government Hospital,Gurgaon>Patwari>Tehsildar>SDM>Hospital>Finally MCG,Gurgaon.

All will take approx. 45-60 working days if you will perform all job personally (collecting & submitting documents by your own). Hope this will helps you.
