Address change and adding spouse name on Passport

Thanks for your quick response friend. one more query to apply for Minor passport proofs to be submitted are
Birth certificate
Father passport
Mother Passport
above docs are enough or do I have to submit any other docs like purpose of applying passport/ any affidavit ?? Please advice?

Hi Narendra,

I have a similar question,
My wife holds passport with her old name(i.e. Name she was having before marriage), she was staying in Gujarat and I am in Maharashtra. Our marriage was held in Gujarat,
now she is staying in Maharashtra. We do have marriage certificate issued by Gujarat Government.

i am planning to take her with me in Qatar as a Dependent, do I have to re-issued her passport with new name and address or passport issued earlier will be Ok. ?

Ketan, without getting her passport re-issued your wife may accompany you as a dependent provided you fulfill all requirements.

Need to renew my wife's expired passport and also apply for new passport for my daughter.

My wife's name in her old passport and in our marriage certificate is XXX YYY (YYY is her father's name). My passport was renewed recently and in that spouse name is endorsed as XXX YYY (as per marriage certificate).

In her AADHAR card and our child's birth certificate, her name is XXX ZZZ (ZZZ being my first name).

Please advice how to proceed while applying for her passport renewal -
Apply with her name as XXX YYY or apply as XXX ZZZ (which is what we prefer) ?
If i apply with her name as XXX ZZZ, will i also have to apply for my passport to be re-issued again to correct spouse name from XXX YYY to XXX ZZZ ?

Your wife will have to decide which name + surname she wants to keep...

She will have to submit a sworn affidavit in the Annexure E format along with self-attested copy of marriage certificate and Self attested photocopy of the spouse's Passport.

For your daughter you will have to submit declaration on plain paper in Annexure D format and a sworn affidavit in the Annexure E format.

Annex E is required only if she changes her name to married name? Not required if she retains maiden name right?

Also, if she goes ahead with married name, is it necessary that i renew my passport to correct spouse name from maiden name to married name? is it ok if i let it remain as maiden name in my PP against spouse name?

Otherwise also Annexure -E is required to be submitted. It has been mentioned for your daughter also.

It is not necessary to renew your passport to correct spouse name from maiden to married name.

Note that from dec 23rd 2016 onward you no longer need.to have your spouse name included on the passport. You can state married status , and juat mention you dont want their details on the passport.
