what is the fee for permit to paro and thimpu and also for making extended permit for places other than these?
Are you Indian citizen? If yes, no permit fees are required for visiting Bhutan. If you are not Indian citizen, I am not sure, but I think you have to take the services of registered Bhutan-based Travel Agents for your trip. I am not too sure about the charges.

No checking except they keep track of the Entry Permit at the Check Post. Make a few photocopies of the Permit, it may be (or may not be) required while traveling inside Bhutan. As far as my knowledge goes, there is no restrictions for Indian nationals to carry INR in Bhutan. In fact, INR looks to be a valid tender in Bhutan. Don't equate other foreign travels with that of Bhutan travel. For Indian citizens, it is almost as if you are in your own country! There may be some resistance at some places (not everywhere though) to accept 1000/- or 500/- rupee notes. Personally, I had no problem, I had also carried much more amount than indicated by you.
