If you use the search engine on this site you will find some posts on Assam.

I have visited the state twice in recent years and steven_ber was there quite recently.

To answer your questions -

You can travel by yourself in Assam, also Meghalaya and Tripura.

No permit other than a valid Indian visa is needed.

You may enter Bangladesh from Meghalaya or Tripura if you have a visa for Bangladesh.

Assam is the safest place to visit while comparing with the other north eastern states. It is the most developed place in the region. But if you want to enter Bangladesh it is advisable to enter via West Bengal whish is near to Assam. Via Tripura and Meghalaya there are border posts but the places in Bangladesh near these two places are not safe at all. I havent been to Bangladesh but I have the knowledge because I am from Guwahati- the capital of Assam. Tripura itself is a very risky place to venture out as the militant organisations are very active there. As far as I know Meghalaya is quite safe but from Dawki which is the border in India is not safe and even in the Bangladesh side also the places are not safe. Those places are inhospitable. I have met a family who had come to visit Meghalaya who came to India via West Bengal only.
