Pundabee is correct in saying that the incidence of malaria is very very low in Kerala such that I do not take anti-malarials when I am there BUT it is up to individuals whether they take them. If you feel it would give you more confidence and peace of mind then why not take them. At certain times of the year the incidence o Dengue fever and Chickungunya is higher than malaria so protection from being bitten is possibly more important, Mosquito nets, DEET etc.)
As for visits you could base yourself in Cochin (maybe Fort Cochin from) where you can visit palaces, got to Cherai Beach, shop in Fort Cochin and Cochin itself, visit the Kathakali and Marital Arts Centres, do a one day trip to the backwaters and/or if you feel you have time spend a night in Munnar. Sometimes less is more if you want to get a feel of an area rather than rushing around in the enclosed space of a taxi/bus or train. |

Not that keen on anti-malarials myself - everybody needs to make a personal decision, of course.
I use a good repellant at mosquito-inclined times and try and cover up a bit. Also, spraying your room with an insect spray an hour or so before bed is a good idea, or, if you have a balcony, a mosquito coil outside is helpful. |
