Regarding Name & Address change in Passport

Hello Sir,

My name is Manisha Vallabhbhai Rathod . I am resides at Mira Road Dist. Thane . from last 20 years .

I got married in 2003 and I got passport in 2004 in my Marries name i.e. Manisha Mukesh Tailor and expired in 2014. However, because under some circumstance I got the divorce after the 6 month only by mutual understanding in my village in Gujarat. I have done the notary ( In Gujarati language)for the same and not the Divorce decree.

And also I have my all the legal documents is on my maiden name only except my passport.

Now I want to apply for change my name and address in the passport with the my maiden name as per above.

I request you to kindly guide me and help me for having the new passport.

May i Inform you that only a decree of Divorce issued by the competent court of law is valid.If you have a paper like a Divorce deed prepared by a notary it carries no value.Technically if you have not obtained divorce from the comptent court of law,the marriage continues to subsist.You cannot re enter your maidan name on the basis of divorce deed attested by a notary because the document itself is not legal.Regarding change of address you can go through similar threads which are already there in this site.
