PP Application for one our year old Kid

Hi experts,

I want to get a fresh passport for our one year old kid, and want to find out the exact application form. Child is born in India and parents live in India as well. (Bangalore)

I have gone through various websites including passport.gov.in, but little confused.

There is one application form for (Apply for fresh PP) where in there are mandatory columns for profession, education qualification etc .,

Can somebody tell me the exact application form that needs to be filled in.
Also kindly let me know how do I register online for an appointment.

No general links please. I only have two questions.

There is a sticky on passport applications in this very subforum. Read that. Also the passport website is now passportindia.gov.in. That and the sticky has all the info you need in an easy to understand form.

Thank you. I had used the same. But what should I do fill in the mandatory columns "Qualification", "profession" etc which is not applicable for the children ?

I hope you are filling in the PDF based Application form for e-form submission. I hope atleast one parent has a passport with spouse name endorsed. Download the application from the home page, Fill it up completely without errors. For Documents required, Click the Documents Required link at the bottom of the Fresh Passport Box on the home page. Then walk thru the options to get the documents required.

Once filled in, there is an option to validate and save. This creates a XML file that you will need in the next step. At this stage you need to create a login in the passport site, Login into the account, upload the XML file created by the PDF (not the PDF itself). Then choose an appropriate appointment for submitting the form and documents. Once you have an appointment, generate the ARN print out. Vivit the PSK with the ARN, Documents and print of PDF filled earlier and fee for passport.

Thanks a lot Narendra and others. I have one last question.
I have filled out the form, but it validates for the Present Address, But I am not able to locate this Present address field in the application form. I have spent lot of time on this. Can some body kindly help me.

You need to activate javascript in your system. then the present adress fields will appear like below when you click on the radio button(is your present address out of India?)

Ah, Thanks Nayan. I do not use Adobe Reader, nor Windows, and was unable to see that form fully. Wonder why even the Govt agencies get locked into Proprietary formats.

I have gone through all the related topics in the forum and found so many useful information. I just want the confirmation for the following :

1) I have left Legal Guradian field blank. Because baby lives with parents. Kindly confirm -> Yes/No
2) I have to take Annexure H along after getting the appointment. Are there any other forms that I should take ?
3) Should the original birth certicate be attested ? How can it be done ? from notary ?
4) Should the photocophy of the passport also be attested ?

I will be truely greateful to the experts and Indiamike forum if my questions are answered.
