Passport Name Issues

Hi,

I have to apply passport for my mother.
Her Name in all her Education documents (10,12,Grad) are Puja Gupta
Looks like she started writing her name as Pooja after marriage. Her Name after that in all documents (PAN/DL/Ration Card) is Pooja Gupta.

I have to fill the name in Passport application, What should I do?

Any comments/suggestions are welcome.

She should first stick to one spelling for her name. The current name would be better.

as narendra said, current name would be better.

My view : she will need to give Affidavit for Puja Gupta & Pooja Gupta are same. I too had this same problem when I applied for my passport.

It would be great if you would have briefed how did you come out of the situation.

My surname was not written on my 10th Certificate whereas my PAN Card, Voter ID was correct with my surname.

When I submitted my papers to the agent, after going through my application, certificates and all, he said that I need to submit an affidavit stating that this & this is same person. Then I made an Affidavit & also gave notice in the newspaper for name change. Punjab Kesri charged Rs.500/- for the same.
