lack of address proof for applying for passports

hi ! i want to apply for passport... but i have lost my voter id, not have a functioning bank a/c and since i live with my parents do not have any utility bills in my name. i do have a UID card which is not being accepted and had a week ago got my name added to my mom's bank a/c as a joint holder... the bank is reluctant to give me a copy of 1 yr statement of my mum's a/c which now has my name too....even her ration card which had my name is lost (is it correct of them to deny me the 1 yr statement? ) ...
m clueless as to how to go about all this... please help!

If you are adult, but dependent on your parents, you may submit affidavit claiming that you stay with your parents.

You would be required to apply for duplicate copy of Voter ID card or you may take a certified copy of voter list from District Election Office in case you are not dependent on the parents.

i have got the one year bank statement of joint hold account with my mother. and my mother's ration card has my name on it . would i still need the affidavit of being dependent on parents in spite of being adult or attested district voter list..... plz help !!
