Get a correct birth certificate from Registrar Death and Birth. Form about 2 Rs Fee about 15 Rs. You have to go to the concerned Registrar (Municipality) where your birth took place. And do not hesitate to go to the one in charge in case it is not issued out after your handed over the form and paid the legal fee to the officer(after some hours or days). My experience: where I went: everything was prepared so that the registrar himself only had to sign things on some afternoons. It was easy to find out his room and approach someone working there and the person told the requirements after I presented my fee slip. And with your birth certificate you get a passport. I would avoid agents in case you are able to stand up for yourself and fill in the required forms...And why do you really need to have your wrong name corrected? It is your legal name: why not get a new passport with your legal name: in case it is really burning because you have a job abroad or something necessarily urgent... In case you go for the birth certificate: bring other documents of family members with correct spelling. And remember: in India, the school leaving certificate might take the role of the birth certificate...

If you use it as a proof for issue of passport then the name problem will not be accepted. You will have to show them address proof with proper name.
But if you have some other proof for address then ration card is not need.
I suggest you get it corrected though. It can become a hurdle anyday.

Why agent? I would meet the officer with all my papers esp. school certificate. The officer should decide and not the agent...
