Can you get me a Sample COVERING LETTER from bank which is to be submitted along with 1-year bank statement.

This standard format is not available with my Bank.

There is no such covering letter, and it is not mandatory to submit 1-year Bank statement. It should be filed only if it carries address of your residence, and you do not have any other proof of residence.

You need to file the following documents as per instructions from Passport office:
(a) Proof of address; (b) Proof of Date of Birth; (c) Citizenship Document (if applicant is a citizen of India by Registration or Naturalization); (d) "Identity Certificate” in original (ANNEXURE B) along with Standard Affidavit Annexure I (required for Government/Public Sector/Statutory body); etc

Provided it carries one year statement including address of your residence (to prove you stayed at address for at least one year). You will also have to submit some sort of photo ID card to prove - you are you!

fresh application

i'm living at my current address which is my permanent address since june/2012. before this i was doing engineering from mumbai and i used to live on rent. so i don't have any address proof of my previous address . i have ration card and 6 months old bank account as address proof of my current address ....HELP !!!

If you can't wait for another one year, you would be required to mention the Mumbai address also, so that police verification could be done there..

You must not misrepresent that you are staying at this address since last six months, even though you do have address proof of the current address of six months.

You may be penalized for stating wrong facts.

For someone who is a college student and is doing it while in a hostel/away from home, it is always better to apply for a passport while still in college. Do not wait to complete college and do not apply immediately after you return home.

It gets un-necessarily complicated.
Follow Prakaant's advice above.

Where can I apply for re-issue of passport?

Hi,

My passport expired more than 3 years ago. My permanent address is in Ahmedabad. I am living in Delhi from last one and half year. In which regional office I can apply? Delhi or Ahmedabad? I have address proof for both, bank statements. Rest of all documents will have Ahmedabad address. Please advise.

In fact, there is a problem. I moved to my current address about two months ago. My bank statments has the previous address. If I get the address changed now, would that work?

Always apply with your current address in Delhi. You will need to fill in both the addresses in Delhi and PC might be done at both places. The Ahmedabad address is history.
