Did you need to complete a concession form?

I've noticed many concession forms in timetables and on websites, but they all seem to be for health or disability concession.

Shaking off very dusty memories of youth I can't recall getting such a form. It could well have been provided at the station as I used an agent in Pune to do my bookings..

Dear sameer,

research Scholars within the Age of 35 Years are offered 50% Concession in Unreserved Second Class (or) Sleeper Class only, when they have to travel on their Research Work. A certificate from the Registrar of the University (or) Director of Admissions would serve the purpose.
