If you are travelling AC, consider just flying. The prices difference can sometimes be not that large.
For shorter trips, just hold it in fer heavens sake. My wife and kids handled 2nd Class sleeper on a few 8 hour trips. Sometimes the toilets were okay, with the help of some Detol wipes and tissue paper. Sometimes they had to hold their breath and get in and out fast. |

India is about 'learning to hold it in'.
That being said 'first class' toilets tend to be very decent and if your riding another just sneak up to the first class toilets. Usually no one will notice. |

Doesn't it depend which class you travel? On an overnight train from Varenasi to Delhi there was a very respectable western toilet plus a shower - that was AC 4 berth sleeper.

My sympathies to your wife. I am a bit of a girly girl myself and the toilets on the trains, and in India overall (other than in my hotels and at my family's homes) were one of the things I really really needed to get over. I went 3rd AC and there was a Western Style toilet, but I went sleeper class and it was Indian Style which I agree seems to be more hygenic than the sit-down types on trains. My advice to your wife is to bring along toilet paper, hand sanitizer, and/or baby wipes. You won't find hand sanitizer in India but you will find tp and baby wipes. Go quickly, hold your breath, don't go when you are stopped, and if you have diharrea just stop it up with immodium and worry about it later. It took me a good week not to hate India for the lack of sanitation in public alone but once she gets over herself and can see past all of that, it will get a lot better. I hope that helps.

Holding it In
Oh and as for the 'holding it in' idea I tried that at first and ended up with a nasty bladder infection - pee when you have to pee. Take along bactrum or another all purpse antibiotic in case a bladder infection, or traveller's diharrea accompanied by fever, hits.

i find the indian loos difficult to navigate with the pants of my salwar down around my ankles, trying to keep them from not slopping in the pee pooling around the foot pedals.
it's a bit like a dance, trying to pee while holding up both pant legs and maintaining a firm foot grip to avoid tipping sideways on a fast moving train.Himalaya Herbals carries a wonderful hand sanitizer that is great to carry on your person whilst in india. it comes in a small pocket size bottle.
