I wasn't really impressed with munnar,so I would go for the 2nd plan. Allepy;kumarkom ar great.Ask him to cut a day at Munnar and instead spend another day at Allepy at kumarkom. And I would prefer Varkala over Kovalam anyday.
You could go Kovalam-Varkala-Allepy-cochin-waynad,preiyar and so on.

Then you should visit Padmanabh Temple in Trivandrum(ask if foreigners are allowed), the Padmanabhapuram Palace,Suchindrum, kanniya kumari. Munroe Island near Kollam is also a beautiful place. I believe Raja Ravi Verma's paitings are also displayed somewhere near Trivandrum in a museum, cant recollect the place, but try to find it.
I havent been to Calicut, but it seems to be popular for its cuisine.

Know I'm just more confused
I looked into Suchindrum and it seems great, just kind a places I would like to visit. To Trivandrum and Kanyakumari I can go from here just between my treatments, so no need to include them to itinerary. It just feels to me that the second choice will be to long end exhausting. But on the other hand the first one is really missing the point of (my) interests. Maybe you can help me with couple of more places like Suchindrum, somewhere between Trivandrum and Cochin (where I'll finish my journey).
And I really appreciate your help.
I've been only to these many places..but I would definitely recomend Allepy,Munroe Islands and Poovar backwaters(which is near kovalam) and Varkala(the cliff) and Padamanabhpuram palace.
Rameshwaram isn't too far away, see if you find it interesting.
There also seem to be many village accomodations in the area, you could check one of those,if interested in staying in rural areas.
