It's starting to look like many people have trouble swallowing pills?
Funny, had never thought of it.
Strange neurosis, you'd think; I mean with modern folk being hung-up about their health, then allergic to the means to address it.
Not making fun of it, really; just peculiar.

As Mach mentioned, not all pills/capsules can be crushed or opened and taken as powder. Some, for instance, are coated to survive stomach acid, if they need to deliver their load further down the system; others incorporate a time-release mechanism in the solid design, and, I guess, a few are just too revolting.
It is a simple matter to discuss this with the prescribing doctor. Although I can imagine some might not have patience with the phobia, I'm sure most will adjust the prescription, or arrange injection or intravenous drip, although the latter would be time-consuming and relatively expensive

I'm guessing... it's not like that, not a question of not knowing how to do it, although there may be a technique to fool oneself.
Imagine the thing you most loathe the flavour of, or that would be most disgusting to eat. This is easy for me, as aniseed and similar flavours qualify: my system just refuses to swallow. It may be psychological, but you just can't.
I guess doctors must be used to encountering such people, just as they will be used to encountering the occasional patient with a needle phobia.

place it way back and wash it down with water. Not swallow and then drink water. that's what i was taught when younger and couldn't/wouldn't take pills.

For me the trick is to take one sip of water in your mouth, don't swallow, then throw the pill in your mouth and drink more water (now swallow). Like this even atomic bomb pills can be swallowed. Looks funny, but works well 
I took only once antibiotics against stomach bugs in India, but they were normal size, nothing extraordinary.
Taking antibiotics with milk is not a good idea, as milk will block the effect.
