Not sick once. We ate at all kinds of places, from highway truck stop cafes to fancy hotel buffets and everything in between, but stayed away from street food (tempting as it was). Drank bottled water, skipped salads, juices, ice and unpeeled fruit. Bottled water for brushing teeth. As vegetarians, we didn't have to worry about the meat.
Yes, a busy restaurant can be a good indicator of fresh healthy food, but one time we stopped in Coimbatore for lunch and ended up at a completely empty hotel rooftop restaurant where we had what was one of the best meals of that trip. Another time in Satna we had some amazing food in the courtyard of a seemingly deserted little hotel.
So, it doesn't have to be a big deal, just use a little common sense.

I hear what you say about bottled water. However, my friends and relatives in India claim to have "filtered water" in their homes, insist that it is "perfectly safe," and get a little insulted when I don't drink it. In my next trip I anticipate that I shall be eating primarily in private homes, so this is not a theoretical question.
