my name is tangella sri kannaka tejaswi as per ssc,inter,aadhr,voter and birth.but in my provisional certificate and cmm it was named as tangella sri kanaka tejaswi. is it creates any problem for passport verification?

Please read the quoted post...

All the addresses should be correctly mentioned in the Application Form, else application may be rejected

All the addresses? i downloaded the form, it just asks for present residential address details.

I will go with the option that my 'current address has been changed'.

Just one doubt, I have been in blore for last 4 months in the last one year(and also one month in the middle of the year). So the address proof that i am showing is photo updated passbook, it should have transactions of just those months i was in blore or a year's transaction?

As soon as you fill "Residing Since " column, a new column will pop up to ask the previous address.

The bank will update passbook as per their rules. You should ask them to enter all the transaction in the passbook.

Oh ok ,thanks for that. I should add addresses for the past one year? If so then its just current address( and Dubai addresaddress. Should that be added too? )

nopes, after i filled residing since april 2015, it didnt ask for any new address.
i then clicked permanent address is different than present address and it asked for permanent address.

no other address is being asked. this is in the pdf that i downloaded from the site, e-form and not online form.

okay, will do that.

i will put residing since 'august 2015' as thats when i am staying in blore. and will take a one year statement in the passbook and my 7 month rental agreement just incase.

i have read at some posts that ppl need to fill up some second form or/and bring a written application too. is that needed in the psk too ?

got an appointment on the 12th of this month. i got the photo updated passbook, psk customer care told me to only bring that and a self attested copy of it.
i am also taking along 8month rental agreement+copy, 8 month bank transaction details+copy, offer letter+pcc requirement mail copy, photo id(pan card)+copy as extra documents. will update after the 12th. i just hope this goes smoothly.

got pcc on the same day of appointment itself. here is the summary.

so i reached on time, security checked my details and let me through(he told others to wait outside who had their appointment time an hour later or so).
got into a queue to get the token , gave the following:
-application form
-passport front two pages, next two pages and last two pages photocopy(showed passport original)
-photo updated passbook first two and next two pages photocopy(showed passbook original)

then there was a problem when he was cross-checking the details, the place of birth mentioned was wrong.(I was sure i didn't make any mistakes but agreed to the mistake). the guy at the counter told me to get a pcc form and a ARN correction form from a nearby counter(2+2=4Rs for the forms). I filled up the pcc form(just have to fill in place where pcc is needed and your name, address. rest all is printed in that form only). and then you dont have to stand in counter, you can go to that guy itself and provide it. Even if you don't do that and wait in the counter, they will call your name in 5-10 minutes as they already gave your file to be created.

So I got a token from them, they added the pcc form and ARN correction form in the file along with the docs i gave above. As soon as i entered the applicant room where all were waiting, i saw my token number popup and went to A counter. This is where they ask for documents and upload it, take photograph, finger print scanner. They made the correction ie place of birth as per passport, and did the rest. And told me to go to B counter as per token number. I went and immediately got the B counter. There they check the authenticity of the docs which was just a min for my case and told me to go to C counter. The C counter is where you get your pcc application granted. I just had a casual talk about me not knowing that pcc would take this much time and thats when the person told me that i would get it in that day itself . I was so happy to hear that and asked if its because the police verification was clear and there is no adverse report , the reason , to which the person said yes. Also I had renewed my passport 2 years back at the psk itself and maybe thats why things went smooth.

But when C counter person was checking details, she said my passbook has only January 2016 transactions and not any of previous, to which i had let her know that my bank only said they can print only january details. and i told that i have bank statements for past one year, to which she said is enough. but i have to get this done from A counter. ie upload the documents. So went to A counter, one of the counter called my name , uploaded the docs. Then went to B counter(yes thats how its done), 10 seconds there, then C counter(to the same person, B counter guy sent me there, which was a very good thing). She told me to wait for 5 min, one person takes my passport and stamps n seals and she signs too. And there I have it, my pcc, on that day itself.

I got back to check the pcc form that i filled and found that there is nothing mentioned like place of birth, where i got it wrong was: the Village or Town or City here refers to the place of birth which should be the same as in passport. (i misunderstood it as current city and entered that hence the confusion later). So thats sorted out.

Important points. The documents required depends on the people at counter B and C mostly. So just make sure you have 2-3 supporting documents though just one is enough . Also be confident while communicating with them and that should solve all the issues. Many people had the docs but got confused when asked question A or B etc. Be courteous to them (they were definitely to me ). If they know that the applicant has all the knowledge of the process then they would just follow along, but don't go overboard and show off or ask questions as to why this is this and that is not that etc, just ask in an informative manner.

And yes, thanks to all the members here who have helped me with this, I appreciate it a lot .

Edit1- The granting officer told me that pcc obtained not via psk is not valid(eg from commissioner office etc). I would have tried the latter if i had a state bank account as i needed pcc urgently but i didn''t know about this info. So yes, always apply pcc for X country via PSK, the proper way to go with it !

I need PCC within 5 days..but appointed is not available before 20th Jan 2016. What to do?
