Address issue for fresh passport of wife

she's not indian national and she stays overseas after 4 -5 months she will add my name in her passport as per tradition of their country she will write MAY CRISTINE GONSALVEZ SINGH
so wen i add her name in my indian passport here in india i want it also to come as 'MAY CRISTINE GONSALVEZ SINGH"

Sorry for goofing it up!

"SINGH" will be added in the passport only if you mention it in last name section of your Application Form.

You will have to submit self-attested copy of marriage certificate along with Application Form.

last section form? what it means dear? there's only 3.4 right? for spouse name?
so in 3.4 i should write MAY CRISTINE GONSALVEZ SINGH?
after her surname i write my surname too? kos that's how i want MAY CRISTINE GONSALVEZ SINGH

You have a column "Spouse's Given Name (Given Name means First Name followed by Middle Name)" where you will have to enter "MAY CRISTINE GONSALVEZ" and then you have a column of "Surname" where you will have to enter "SINGH".

ah ok thanks dear that's what was confusing me a bit
i was thinking i have to write MAY CRISTINE
and then in surname section GONSALVEZ SINGH
but what u say seems more better MAY CRISTINE GONSALVEZ and in surname i write SINGH
so just want to ask that ur totallly sure about writing this way? and 2nd when they will re-issue my passport how will my wife name be written on psassport
MAY CRISTINE GONSALVEZ SINGH? right? kos that's the way i want , thanks a lot dear

Am not an authority on Passport but logically speaking it suits fine.

If there is any error in the order, the authorities in PSK will help you in correcting it...

dear prkant I asked passport seva helpline also he didn't have much idea of it either but when i asked him is it better to write this way write MAY CRISTINE GONSALVEZ and in surname column write SINGH? He agreed so that's how i will write then i wish when they print they print it as MAY CRISTINE GONSALVEZ SINGH kos thts the way we want it
at first he was asking me for gazzeted name change etc
but i said she don't want to add my name in her passport neither she's indian national
i think that name change annexure etc applies to wife when she want to use husband surname
but in my case i am Husband just want to give surname to my wife name in my passport
so it's different thing all together

why passport application form doesn't save the data? it says you can't save? only print

Passport Seva Helpline is basically a call center (run by third party) to impart routine information and they won't be of much help to you.

Gazzetted name change, afidavit etc won't be required even for a lady using husband's surname.

You should download latest form from official website, and complete the form. Application Form saves data and you may proceed where you left last time.

Once completed you should Save & Validate before uploading the form.

prakant i found 3 kind of application form one from https://portal2.passportindia.gov.in...ct/welcomeLink website which form is in boxes and not offering option to save then i found another from http://passportindia.gov.in/AppOnlin...line/printForm it says download main applicatio form for fresh or reissue but when i fill this it gives no option to write wife's name.
please guide me the link if u have i m confused with what form? kos there are 2 send me a link to download

Please visit official website of Passport Seva.

And click on Forms and Affidavits. First option is "Download e-Form ".

You will reach here.

Please download form meant for "Fresh or Re-issue of Passport".

Please note, you will have to download soft copy of eForm, fill the same off-line and then will have to upload it back.

Further please note, the spouse's name option will appear below mother's name when you select Marital Status (as Married).

prakant - should i use same email id? that i used about few years back for passport or should i use new email id? for this time? i mean while registration for loging in for uploading form etc?

prakant if i get appointment of thursday , and suppose if it comes in 3 days so when passport will come? saturday sunday they r closed right? i haven't taken the appointment yet after i register can i wait for later date? for appointment
